TITLE:  Client Services Coordinator

 

POSITION:  Full-time, regular, exempt

 

SUPERVISOR:  Associate Executive Director

 

SUMMARY

 

The Client Services Coordinator contributes to AVOL’s mission of Ending HIV in Kentucky by providing a wide range of client services to increase housing stability for persons living with HIV/AIDS (PLWHA) and who are housing vulnerable and/or homeless.  For PLWHA, housing is one of the strongest predictors of access to treatment and health outcomes including viral suppression.  The Client Services Coordinator understands that housing is treatment and works with PLWHA to access and maintain stable housing and connection to health care services. The position is part of AVOL Kentucky’s comprehensive team working across a continuum of services that meets people where they are at. The continuum includes testing, connectivity to care, needs assessment, emergency services, referral, empowerment services, emergency housing and longer-term housing solutions. 

 

KEY RESPONSIBILITIES

 

Client Services:

 

Provide full range of client specific social work/case management activities to facilitate housing stability and viral suppression: intake and assessment, case planning, referral, liaison/brokering, education and advocacy.  Case management activities include but are not limited to:

  • Evaluate clients housing needs and provide timely client assistance in accordance with AVOL’s written policies & procedures for client services. 
  • Provide client support services in a timely and accurate manner.
  • Assess, teach, and guide clients in setting realistic goals to stabilize their housing and health. 
  • Conduct home visits with clients as needed. 
  • Conduct or arrange housing inspections as needed.
  • Transport clients as needed.

 

Through direct linkage, advocacy, education, and support, implement with the client a case management plan to secure and/or maintain access to and use of: 

  • Community social services and resourcesEntitlements, public assistance, housing, financial assistance, employment, education and legal (civil and criminal).
  • HIV/AIDS services, education and resourcesprimary and specialized medical care, dental care, Ryan White services, health insurance, hospice/home health, treatment education/adherence counseling and substance abuse/mental health therapy.
  • Secure and maintain needed program(s) eligibility documentation for assigned grant-funded programs.
  • Maintain needed documentation of services rendered to include: contacts, units, progress notes, assessments and outcomes.
  • Assist with facilitation of workshops and social events for AVOL clients as needed. 
  • Respond to walk-ins and phone calls for individuals seeking services. Document needs not met. 

Identify, establish and/or maintain contact with key housing, medical and social services providers in the community, specific focus to mental health and addiction providers. 

  • Participate in training, forums, professional committees, etc. when appropriate to stay abreast of the latest trends and best practices in service for clients living with HIV/AIDS.
  • Represent AVOL Kentucky at meetings, health fairs, etc. to help promote AVOL’s mission and specific programs as needed.
  • Maintain work consistent with agency and grant standards, and adhere to agency/grant policies and procedures.

·       Maintain AVOL’s reputation as experts in community engagement, housing services, HIV support and services, and other health services.

 

·       Participate in agency meetings and events and collection of data/information. 

 

·       Perform other duties as designated by supervisor or Executive Director. 

 

Required Qualifications

 

  • Bachelor’s degree in social work or a related field. 

 

  • Excellent verbal, written, and interpersonal skills. 

 

  • Basic computer skills. 

 

  • Valid KY driver’s license. 

 

  • Personal vehicle must meet minimal safety standards and auto insurance must be maintained at all times. 

 

Preferred Qualifications

 

  • Experience with HIV/AIDS, homelessness, housing, substance abuse, mental health and/or residential programs.

 

  • Bilingual in English/Spanish. Eligible for 2.5% differential in pay.
